In The Patient Episode 4, we see Sam’s captive being introduced to the audience as Ellias. The entire episode covers the struggle Sam has with killing his prisoner and at the same time listening to his other captive, his therapist- Alan and the struggle all these characters face with compliance.
The Patient is created by Joel Fields and Joe Weisberg with the executive producers being Joel Fields, Joe Weisberg, Steve Carell, Caroline Moore, Victor Hsu and Chris Long. The production companies for the series include Joel Fields Productions, Flim Flam and FXP. The main cast features Steve Carell as Alan Strauss, a therapist and Domhnall Gleeson as Sam Fortner, a serial killer and the new patient of our therapist in question.
Actors Linda Emond plays Candace Fortner along with Laura Niemi as Beth Strauss, Andrew Leeds as Ezra Strauss and David Alan Grier as Charlie Addison. Alex Rich is also part of the cast. The Patient is slated to be ten episodes long with the third episode having a runtime of 23 minutes.

The Patient Episode 4 Ending Explained
Unlike its predecessors, The Patient Episode 4 does not rely on a cliffhanger ending but, rather leaves us with a sad tone and bittersweet anticipation of what happens next. The episode follows Sam’s struggle with keeping his urges in check. The first thing we witness in the episode from Sam’s end is a confrontation he has with Alan about how he can keep his killing impulse under control.
The new captive is revealed as Ellias and Sam is ready to take his life. However, Alan formulates a plan to keep Sam busy and distracted and asks him to go about his normal day by being at work and trying to distract his mind. As Sam is away, Alan strikes up a conversation with Ellias, who is blindfolded and trapped in the other room.
The two talk throughout the day and share things about each other. However, when Sam returns from work at night, it is apparent that he would just get downright to killing Ellias. Alan suggests Sam meet with his ex-wife and as surprising as the suggestion is, it might be able to take Sam’s mind off killing.

After Sam departs, we see a weak Ellias crying and shivering for the fate of his life. This is when Alan starts singing Country Roads by John Denver to help Ellias with his anxiety and worry. Ellias soon joins Alan in his melody and that is how The Patient Episode 4 comes to an end.
